Which job is this for?

Your code covers more than one of this contractor's projects.

Pick the project these hours were worked on. The rates and classifications are different on each, so this decides what your payroll is checked against.

Export from QuickBooks, ADP, Paychex, Paycor or Gusto and we map the columns for you — or download the blank template. Whichever you choose, you see everything on screen and can fix it before anything is filed.
Monday–Sunday is THIS JOB ONLY. A certified payroll covers the site of the work, so hours on your other jobs do not belong in those boxes. Put them in Other hrs instead, with the rate you paid — overtime is owed on hours over 40 across a worker's whole week, so someone with 30 hours here and 20 elsewhere is owed overtime even though this job never reached 40.

Sign the Statement of Compliance

This is the certification on page 2 of the WH-347. Read all four statements — you are certifying each of them, and falsifying any of them is a criminal matter.
